Increments of $10,000

Guaranteed Issue up to $150,000 for Employee, $30,000 for Spouse/Domestic Partner

Evidence of Insurability required if enrolled or increased after initial orientation

Dependent Term life – One premium for all eligible children under age 26

Three levels available:

$5,000 = $1.00 per month

$10,000 = $2.00 per month

$15,000 = $3.00 per month

Employee pays 100% - Automatically deducted through payroll deductions
